Forms We Use
We have made many of our manual forms publicly available so you can see exactly how we work, from onboarding through to service changes.
Most of the forms we use day to day are built into the software we use to manage participant services, so you may notice small differences in formatting. The questions and purpose are the same, the platform just presents them a little differently.
Sharing them means participants can see what to expect before filling anything out, and other providers can find ideas to improve their own systems. Like our policies, these forms keep evolving.
